Kannact’s unique system of chronic disease management helps patients effectively track and manage key information related to their chronic treatment.
By making this data readily available to patients and other healthcare providers, the Kannact system enables chronic patients to maintain optimal health and enhance their quality of life, while at the same time minimizing the cost of treatment.
The system is available at no additional charge to the patient and reduces costs for the insurers.
This app is used by Patients registered with Kannact Inc, the following are the functional areas of the app.
1.) Login
2.) Message: Patient sends messages to Providers (RN), messages posted by a provider are viewed by Patients.
3.) Track Biometrics:
a.) Support for Medical Devices to transmit various Chronic data points to
Kannact Cloud.
b.) Manual Entry: Patients can enter manual readings for various Chronic data points.
b.) View Results: Patients will be able to view their Chronic data in Chart(7,30,90 days)
and view/download there reports.
4.) Learn: Provider assigns Education content (PDF, Video and YouTube Channel links)
which is viewed by a patient.
5.) Patient Care: Provider posts the Instructions to the Patient
6.) Surveys: Provider assigns surveys on demand
7.) Wearables: Prompts the user for permissions and on accept post the wearable data
from Health Kit to Kannact Cloud
